Woolfe

Woolfe was a half orc fighter who wore a necklace of missiles. When the item failed its saving throw vs. fire, there was a large explostion which decapitated Woolfe and killed most of his companions during the Expedition to the Barrier Peaks (https://en.wikipedia.org/wiki/Expedition_to_the_Barrier_Peaks).
